Service Coordination


The Service Coordination team undertake assessment of tangata whaiora/consumers that may require additional environmental or social or independent living support in the community to facilitate recovery.  Staff in the team develop support plans in line with peoples' goals.

The team also make referrals to appropriate support agencies such as supported accommodation and residential services aimed at assisting people to achieve a level of recovery that supports independent living.

Part of a package of care may include support in the community where support workers work individually with people to increase independence and include home based support packages for short term additional support to assist individuals to manage at home.

Acute packages of care are developed to support acutely mentally unwell clients, generally in their own homes.

Enhanced community support, which is intensive support up to 2-3 times daily during periods of acuity for up to 12 weeks to assist individuals during periods of acuity, predominately supports people with medication management, but can focus on other intense needs as well.

Referral to the service is by internal referral from mental health and addiction services.
